Understanding the Pocket Eye Chart and Visual Acuity Testing

Visual acuity (VA) assessment is made simple with this pocket vision screener. This tool is specifically designed to provide a distance equivalent VA even when held just 14 inches from the patient's eyes. To perform a vision test, follow these steps:

  1. Have the patient wear their corrective lenses, if any.
  2. Ensure the card is evenly illuminated.
  3. Instruct the patient to hold the test card 14 inches away from their eyes.
  4. Ask the patient to cover their left eye (always examine the right eye first).
  5. Ask the patient to read each letter or word on the smallest legible line.
  6. Record the VA for the right eye using the accepted notation method.
  7. Repeat the process for the left eye and then with both eyes.
  8. Record the VA for each eye condition.

For patients who cannot see the letters or numbers, record their vision as count fingers (CF), hand motion (HM), light perception (LP), or no light perception (NLP), along with the corresponding distance.
